Release of Liability:
I, the undersigned parent or legal guardian of the child signed up, understand and acknowledge that there are dangers and risks to which my child may be exposed by my child’s participation in sports, and other activities provided by the Idaho Youth Sports Commission and their staff. I, on behalf of myself and my child, hereby irrevocably agree and without reservation of any kind to assume all risks of injury arising out of or related to my child’s participation in any such activities, regardless of the cause, including risk of death. I, On behalf of myself and my child further hereby agree to forever and irrevocably discharge, release and hold harmless Idaho Youth Sports Commission, its owners, officers, board members, agents, and their respective family members from and against any and all liabilities arising out of or related to my child’s participation in such activities in Plummer Challenge event, and hereby forever and irrevocably waive any and all actions, suits, claims, demands, and or causes of action arising out of or related to my child’s participation in any such activities, including those arising from or related to injuries sustained as a result of negligence. I acknowledge and agree that I have read and understand this liability release and waiver and voluntarily enter into it without any reservation whatsoever. No representations have been made to me or my child than those contained herein and this Liability and Release shall bind myself, my child, and our respective heirs, executors, and administrators, and assignees.

I hereby consent and without further consideration or compensation to use ( full or in part) by Idaho Youth Sports Commission its owners, board members, staff, and agents of all pictures, videotapes, or voice recordings and/or written extraction, in whole or part, for the purposes of illustration, broadcast, or distribution in any manner. I understand that any personal information regarding my age, name and address will at no time be published, printed, or shared.
